Job Description Job Description Job Title: Nurse Practitioner – Primary Care Department: Primary Care / Family Medicine Reports To: Medical Director / Supervising Physician Position Summary The Primary Care Nurse Practitioner (NP) provides comprehensive, patient-centered healthcare services across the lifespan. The NP assesses, diagnoses, treats, and manages acute and chronic medical conditions while promoting preventive care and health education. This role emphasizes collaborative practice, continuity of care, and evidence-based clinical decision-making. Key Responsibilities Clinical Care Conduct comprehensive health assessments, including medical history and physical examinations Diagnose and manage acute and chronic illnesses (e.g., hypertension, diabetes, asthma) Order, perform, and interpret diagnostic tests (labs, imaging, EKGs, etc.) Prescribe medications and treatments in accordance with state regulations Develop and implement individualized treatment plans Perform minor office procedures (e.g., suturing, wound care, I&D, joint injections) Provide preventive care services including screenings, immunizations, and wellness exams Patient Education & Counseling Educate patients and families on disease prevention, health maintenance, and treatment plans Counsel patients on lifestyle modifications and risk factor reduction Support chronic disease self-management Care Coordination Collaborate with physicians and interdisciplinary healthcare team members Refer patients to specialists when appropriate Coordinate follow-up care and ensure continuity of services Documentation & Compliance Maintain accurate, timely electronic medical records (EMR) documentation Ensure compliance with HIPAA and regulatory requirements Participate in quality improvement initiatives Qualifications Education Master’s or Doctorate degree in Nursing from an accredited program Certification & Licensure Current, unrestricted Nurse Practitioner license Board certification (FNP, AGNP, or related primary care certification) DEA registration (or eligibility) Current BLS (ACLS preferred, depending on setting) Experience 1–2 years of primary care experience preferred (new graduates may be considered in some settings) Knowledge, Skills & Abilities Strong clinical assessment and diagnostic skills Knowledge of evidence-based primary care guidelines Excellent communication and interpersonal skills Ability to work independently and collaboratively Proficiency with electronic health records (EHR/EMR systems) Commitment to patient-centered care Work Environment Outpatient clinic setting May require occasional evening or weekend hours Prolonged periods of standing and computer use
