I. Basic Purpose:
This position functions data collection, reporting and analysis
for assigned database. Responsible for collecting accurate,
complete and timely data to registry platform; submission of data;
coordination of data collection, abstracting patient information
and outcomes, documentation, and communicating results through
reports and direct contact with departments, physicians, and staff
for the purpose of improving quality of care and documentation.
This position is eligible for telecommuting, however requires
scheduled meeting attendance and improvement activities as assigned
by the direct supervisor.
II. Major Duties and Responsibilities:
Complies with established corporate and departmental policies,
procedures, objectives, quality assurance methods, and safety
codes. Demonstrates compliance with licensing, regulatory and
accrediting agency provisions as required.
Consistently demonstrates appropriate and professional
communication behavior toward patients, customers, and coworkers.
Cooperates with Management and peers to promote an environment of
teamwork and collaboration.
Reviews patient records for database indicators. Performs
concurrent and retrospective medical record data abstraction for
internal and regulatory mandated initiatives.
Ensures database entries are complete and accurate.
Documents and maintains records of database.
Submits data within specified time frame.
Performs timely follow up with patient or family for data
collection purposes as defined by database quality metric. Record
of this follow up will be kept electronically for reference.
Ensures data abstraction is complete within 1 months of patient
Responsible for data entry: analyzes, interprets, formats and
presents clinical data for a variety of users; identifies services
and/or cases for clinical studies and may support analysis of

Maintains HIPAA regulations and standards.

Candidate should have experience with data collection and
maintenance, including but not limited to: medical billing,

relations. Knowledge of medical terminology, basic anatomy,
physiology, and disease process required. Ability to prioritize
multiple tasks and responsibilities, work independently with
minimal direction and strong organizational skills are

based applications (Microsoft applications) with competence in word
processing and database management, PowerPoint and spreadsheets.
Candidate should have some experience gathering information in a
complex hospital system environment. Excellent written and verbal
communication skills are required.
