Duties and Responsibilities:
This is a professional LPN position with major responsibilities
in the provision of health services for Communicable Disease. In
addition, provides nursing coverage in the HIV/STD, and Family
Planning clinic when needed. This position is under the supervision
of the Medical Director., The incumbent in this position works
primarily in a clinic facility with service provision focuses on
the Tuberculosis Program, and other Communicable Disease programs
including HIV/AIDS, Sexually Transmitted Infections.
Organizes implements and conducts Tuberculosis investigations.
Advises clients on care and management of Tuberculosis diseases,
prevention, and treatment. Applies case management principles to
all cases of Mycobacterium Tuberculosis, suspect cases and
tuberculosis infection. Acts as lead coordinator and liaison with
external agencies and hospitals linking clients to care. Assists in
the investigation of reportable communicable diseases when
necessary. Provides with direct observed therapy (DOT) of TB
Updates staff on new changes to TB protocols or referral
process. Participates in regular staff meetings for information,
planning and review of assignments. Assists in the orientation of
new staff members and monitors the DOT position for TB
Conduct programmatic functions for HIV/AIDS, and, STD, Martin
County Health Department, assist with DIS functions Conduct
Interviews, and re-interviews on clients with STD infections for
treatment referrals, and partner notifications. Documenting each
activity conducted with date and time in HMS System. Educate
Patients regarding STD infections and appropriate medical care.
Interviewing HIV positive clients, referring them for linkage to
follow up Medical care.
Conducting cluster activities and condoms distribution.
Maintaining individual blood kits. STD testing on clients.
Shipping specimens to appropriate labs following shipping
Infectious and Biological.
Provides immunization to children and adults who qualify for the
VFC, 17 programs and inputs immunizations and vaccine information
into Florida Shots the same day. Provides education to parents,
children and general public regarding the benefits of
immunizations, contraindications and relief measures following
administration of the vaccines.
Performs other related duties as required in support of the
Agency's vision and mission. Participates in regular staff meetings
for information, planning and review of assignments, conferences
and training for communication purposes and personal development.
Participates in Quality improvement activities.

duty required of the incumbent includes working in special needs or

but not limited to, responses to or threats involving any disaster

Knowledge, skills and abilities, including utilization of
equipment, required for the position: Advance to intermediate
computer (Microsoft Word, Excel, E-mail and internet),Ability to
establish and maintain effective working relationships, customer
service skills, communicate verbally in a courteous and effective
manner. Ability to plan, organize and coordinate work assignment
for self. Ability to retrieve, interpret and disseminate health
information data. Ability to document history in patient chart.
Knowledge of nursing principles, techniques, and scope of practice
of the Licensed Practical Nurse. Knowledge of Electronic Health
Licensure/registration/certification requirements (If
applicable, list the appropriate Florida Statute or federal
regulation cite): Licensure as a Practical Nurse in accordance with
Florida Statue 464 or eligible to practice nursing in accordance
with Florida Administrative Code 210-8.22 or 8.27, Drivers
