Job Description SummaryThe Care Coordinator will be responsible developing a long-term relationship with our patients to effectively enroll them in the Link program or refer them to a participating provider. This role will serve as a communication liaison for the Territory Manager, Physician Office and Patient. We have over 65,000 employees and a presence in virtually every country around the world to address some of the most challenging global health issues.PRIMARY DUTIES AND RESPONSIBILITIESArticulate the features and benefits of Home Care product line for Urology with additional responsibility for ancillary urology product line features and benefits. This includes responsibility for knowledge relative to all aspects of use and maintenance of medical supply equipment.Complete initial intake and follow-up calls with patients via inbound and outbound calls/dialer to include identifying and resolving patient concerns within the process or product.Work in conjunction with the designated admin support team to ensure all pending documentation is completely accurately.Coordinate the communication needed to request document revision with the Patient, Territory Manager and/or the Physician office.Documenting the patient interaction in Salesforce by updating the patient account with call notes, updating account status, and providing reference notes to external teams.Communication with the Territory Manager also includes e-mails and follow-up phone calls outside of the Salesforce system.Maintain departmental call volume requirements to include inbound and outbound calls.Maintain a positive work atmosphere that embodies BD Home Cares' philosophy- Professional excellence, teamwork, and integrity.Maintain an exceptional level of customer service coupled with professionalism during each interaction with the Patient, Territory Manager, Physician office and fellow associates.Meet or exceed monthly departmental goals as established by Sales Ops management.Adhere to phone activity/compliance standards to include Medicare guidelines, HIPAA, ACW, and minimum call volumes.ANCILLARY DUTIES AND RESPONSIBILITIESMaintain high attention to detail throughout daily tasks and projects including accurate notation and details of each customer account to ensure proper and timely follow up.Assist other departments and team members when required or requested to help meet business needs.Seek continual education opportunities to remain up to date with products and processes.EDUCATIONHigh School Diploma or general education degree (GED); one to two years related experience and/or training; or equivalent combination of education and experience.Ability to read, analyze, and interpret medical supply publications, technical procedures, and/or training tools. Ability to write internal and external business correspondence.Ability to effectively present information and respond to questions from management, team members, and/or customers.Ability to calculate figures; ability to apply intermediate math skills. Ability to solve practical problems and deal with a variety of variables. Knowledge of Microsoft Office Applications, Access or other database software.
